Email or fax a copy of your completed 222 form to vetsales@pharmsourceah.com or (754) 200-2905 (fax).

Our Controls/Compliance team will review your form for any errors.

Label and mail form
Our team will email you an overnight shipping label to send your form in after it is approved. Cost: $20 per label.

Processing and packing
Once we receive your 222 form, we will process and pack your C2 order.

Scope
Paper Form 222 orders only
Rapid C2 Service applies to Schedule II orders submitted on a paper DEA Form 222. Orders signed electronically through CSOS do not use this service. Schedule III through V medications need no Form 222 and are ordered through the portal or your account manager.
